By helping you to monitor your PT/INR values and adjust your Warfarin (Coumadin) therapy, the Anticoagulation Service at Holyoke Medical Center is here to make sure that you stay in the best of health, in the most convenient way possible.

All it takes is a finger-stick to draw a drop of blood to find out your PT/INR, and to make sure your Warfarin dosage is correct.

Our highly skilled registered nurses maintain certification in Anticoagulation Therapy Management. They work closely with you and your physician to establish and maintain your prescribed blood level of your anticoagulation medication.

On the first visit, your nurse will take a thorough medical history, perform an initial evaluation, and begin to teach you how to live safely, and actively, with your anticoagulation therapy.

The Anticoagulation Service is located on the 4th floor in the Main Building on the Holyoke Medical Center campus. Hours are 8:00 AM to 4:30 PM, Monday through Friday. A prescription from your primary healthcare provider is required. Please call us for an appointment at (413) 534-2834.
