What is a Community Health Needs Assessment (CHNA)?

A CHNA is part of the hospital’s strategic plan that evaluates the overall health of the community. The CHNA identifies recurring causes of poor health within the communities we serve, and then focuses our resources to support and drive positive change with the identified barriers.

The Patient Protection Act of 2010 requires tax-exempt hospitals to create a hospital community health needs assessment every three years. The hospital CHNA is developed alongside community stakeholders, community residents and community based organizations.

Why should I participate?

It is important to have your voice included and to have your opinion heard. This survey gives you an opportunity to share what health challenges and struggles you, or a loved one, have faced. Holyoke Medical Center will then create ways to address the most outstanding needs with added resources and community education.

Who else will be involved with the CHNA?

Community Based Organizations, city and state officials, and local leaders will also share their knowledge, in-sight and concerns of the most pressing health concerns they witness in our community.

Data is also gathered at a local, state and federal level to better understand current health trends, the health status of our community, and to highlight important issues that should be addressed and considered as part of our strategy.


What will be done with the information?

The information collected from residents, stakeholders, community organizations and health care trends will be reviewed, analyzed and put into an implementation strategy to be published on the medical center website at HolyokeHealth.com. The strategy is then followed for three years in an effort to contribute to a healthier and happier community.
