A majority of the treatments available at HMC Family Physiatry are covered, and our team will obtain appropriate authorizations from insurance provider(s). If something is not covered by insurance, our team will notify you before proceeding with the treatment.
Do you treat sports-related injuries?
Yes, our board certified team has extensive experience treating sports-related injuries.
What does a new patient need when visiting the office for the first time?
Please bring a form of ID and your insurance documents. First time patients at HMC Family Physiatry are asked to arrive 30 minutes before their scheduled appointment time to complete an intake questionnaire.
