As an independent community hospital, Holyoke Medical Center is committed to addressing the most pressing healthcare needs of those living and working in the surrounding area. Every year the hospital provides expertise, time, and resources to improve the health of all people in our community.
Community Health Needs Assessment (CHNA)
Every three years Holyoke Medical Center conducts a CHNA, in order to better understand the needs of the communities we serve. Once the CHNA is complete, an effective implementation strategy is created to address the most pressing issues for which the hospital is able to assist, in collaboration with many community organizations.
2022 CHNA Planning Process
The process for gaining community input will include a number of community conversations, stakeholder meeting and an extensive community survey that will happen over several months. All materials will be provided in English and Spanish.
